Primary Purpose
Provide direct care
to residents of the skilled nursing facility in accordance with standards
of practice and regulatory requirements of the Department of Health Services.

Standards
Essential Functions
Quality of Care
- Ensures quality
nursing and wellness care to the residents in the SLV SNF
- Supports the SLV
Mission and practices the philosophy, nursing objectives and standards
of the Department of Health Services.
- Exhibit behavior
as set forth in the ESC Core Performance Standards (see attached)
- Assists residents
in the activities of daily living, transfers and mobility
- Encourages resident
provides transportation for residents to participate in activities
- Maintains order
and safety in resident living areas
- Assures maximum
functioning of resident through ROM activities, toileting schedule,
feeding and turning schedule
- Reinforces training
as outlined by therapy staff
- Observes residents
for any physical and behavioral changes in condition and reports to
the charge nurse
- Maintains consistent
attendance and punctuality; follows SLV policy and procedure for time
off requests and sick call.
- Demonstrates professional
demeanor and appearance
- Is a mandatory
reporter for all suspected abuse situations
- Complies with
all HIPAA standards of practice
- Other duties as
assigned by charge nurse
Human Resource
Development
- Maintain active
CNA certificate and participate in inservices provided by SLV
- Participate in
all staff meetings
Communication
- Records all pertinent
information pertaining to the daily care of residents including weights,
ADLs, I & O and reports to charge nurse
- Records resident
inventory and labels all resident property
- Demonstrate ability
to work cooperatively with co workers, other staff and supervisors
Safety
- Reports all accidents
and unusual occurrences immediately to charge nurse
- Follow all established
safety procedures and precautions when operating equipment and report
all unsafe/hazardous conditions to charge nurse.
- Observe and report
any needed repairs or service for building or equipment to charge nurse
- Participates in
Facility Emergency Plan training and fire drills, and in an emergency
or disaster occurs, immediately assumes the pre-assigned emergency duties.
Required Qualifications:
- Current California
CNA certification
- Current CPR certification
- High school diploma
or equivalent
- Ability to read,
speak well and be understood in the English language
- Satisfactory completion
of physical examination including TB test
- Fingerprint clearance
from Department of Justice
Preferred elements:
- At least 6 months
experience as a CNA in acute or long term care setting
- Bilingual ability
Exposure risk:
The CNA is at high
risk for exposure to blood and body fluids
Physical/Sensory
Requirements: (with or without corrective devices)
- Able to move continually
throughout the facility during the work day
- Able to cope effectively
with physical and mental performance elements
- Demonstrate emotional
stability
- Able to lift,
push, pull up to 35 pounds
- Dexterity to pinch
small objects
- Vision adequate
to read medical instruments, medical record
- Hearing adequate
to communicate with peers and residents
