Job Title:  Activities-Skilled Nursing Facility, St. Paul's Towers

Department: Health Services
Reports to: Director of Nursing
FLSA Status: Non-Exempt

Primary Purpose:
Responsible for the planning, developing, organizing, implementing, evaluating, and directing of Activity programs in the Skilled Nursing Unit and assisting staff, residents and resident’s family to understand and cope with a resident’s facility, while ensuring that the needs of the residents are met and maintained in accordance with current existing federal, state, and local standards as well as our established policies and procedures and ensuring that the spiritual development, emotional, recreational, and social needs of the residents/patient are met/maintained on an individual basis.

Job Performance Standards

I. CORE PERFORMANCE STANDARDS: Exhibits behavior as set forth in the attached Core Performance Standards.

II. ESSENTIAL FUNCTIONS:

Plans, develops, organizes implements, evaluates, and directs the Activity Programs to ensure all resident/Patients assessed needs are met. Assists in developing and implementing policies and procedures for identifying the spiritual, social, recreational and emotional needs of the resident/patient.

Develops activities plans, including goals and treatments for each resident who needs such services with input from the resident, the family, the residents care physician, and the interdisciplinary team.

Assists in the development, administering, and coordinating of department policies and procedures and supervising Activity Assistant and volunteers.

Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations on changes in policies and procedures to the Administrator

Assists in development of the department budget and monitors expenditures to stay within that budget.

Assists in the development of and participates in regularly scheduled orientation and in-service training programs in relation to the social, emotional and recreational needs of the residents/patients.

Interviews residents/patients/families as necessary and in a private setting, and involves residents/patients/family in planning activities when possible.

Assists in arranging transportation for field trips and to other facilities when necessary.

Serves as an advocate for the resident and is notified and follows up with any issues related to the psycho-social well-being and mood state of each resident.

Completes sections N1, N2, N3, N4, N5 of the MDS promptly and completes the RAPS by the due date.

Interviews and provides a written assessment and care plans of each resident within seven days of admission.

Documents written progress reports in the Activities sections of the resident's health record on quarterlies, changes of condition, or "as needed" progress reports.

Reviews the admission agreement paperwork with the resident and/or responsible party within seven days of admission.

Records and maintains activity progress notes as well as a record of resident/patient activities. Compiles and maintains an activity schedule, resident attendance records, available projects, crafts, resources, etc.

Participates as a member of the Interdisciplinary Team and in regular staff. conferences.

Is available to meet with residents and families, as necessary, in a private setting. L. Participates in all care plan meetings.

Assists in arranging transportation, adaptive equipment, and clothing necessities.

Participates in discharge planning.

Conducts in Resident's Council monthly meetings

Maintains consumer information boards appropriately and ensures resident's rights are posted and reviewed regularly.

Reviews complaints and grievances and makes necessary oral/written reports to the Director of Health Service and/or administrator as appropriate.

Provides consultation to members of staff, community agencies, etc., in efforts to solve the needs and problems of the residents through the development of the Activities Program.

Keeps updated on current Federal and State regulations, as well as professional standards. Makes recommendations on changes in policies and procedures to Health Services, as well as assisting in the development, administration, and coordination of department policies and procedures.

Reports all incidents/accidents immediately and reports all unsafe/hazardous conditions/equipment immediately. Ensures that established safety regulations are followed at all times.

Participates in discharge planning, development and implementation of activity plans, schedules and resident/patient assessments.

Participates in the overall quality assessment and improvement of programs.

Participates in Facility Emergency Plan training and fire drills, and if an emergency or disaster occurs, immediately assumes the pre-assigned emergency duties.

Shall work well under pressure, meeting multiple deadlines. Shall at all times demonstrate cooperative behavior with staff, colleagues and supervisors.

Education Requirements/Experience

The Director of Activities must meet one of the following requirements in each area:

  • Activities: Have two years of experience in recreational program within the past five years, one year of which was full-time in a patient activities program in a health care setting.
  • Be a certified occupational therapist, art therapist, music therapist, dance therapist, recreation therapist or occupational therapy assistant.
  • Have satisfactorily completed at least 36 hours of training in a course designed specifically for this position and approved by the Department of Health Services.

Exposure Risk:

The Activities Director is at minimal risk for exposure to blood and body. Fluids

Physical and Sensory Requirements: (With or Without the Aid of Mechanical Devices)

  • Must be able to move intermittently throughout the work day.
  • Must be able to speak, read and write the English language in an understandable manner so that the essential functions of the job can be performed satisfactorily.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to function independently, have personal integrity, flexibility, and the ability to work effectively with residents/patients, personnel and support agencies.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to relate and work with the disabled, ill, elderly, emotionally upset, and at times hostile people within the facility.
  • Must be able to lift equipment, supplies, and occasionally move furniture.
  • Must complete physical examination and TB test.
  • Employment contingent upon fingerprint clearance from Department of Justice.
