Job Title: Case Manager, St.
Paul's Towers
Reports To: Director of Outpatient Services
Department: Health Services
Hours: 9:00 am – 5:30 pm, every weekend and on-call
Primary Purpose
Provide nursing assessment, service coordination, and case
management to Independent Living and Assisted Living Residents to
promote wellness and maintain independence. Receives administrative
direction from the Director of Outpatient Services.

II. Essential Functions:
Quality of Care
- Ensures quality nursing and wellness care to all residents of
Independent/Assisted Living
- Supports the SPT Mission and practices the philosophy, nursing
objectives and standards of the Department of Social Services
- Exhibits behavior as set forth in the ESC Core Performance
Standards (see attached)
- Establishes and conducts initial assessment process upon
admission to SPT; evaluates health status of case load on an ongoing
basis
- Provides professional and crisis intervention to the residents
in cases of emergencies
- Encourages individual residents to participate in their own
wellness plan and assisting in the development of that plan
- Provides accurate documentation for each resident on caseload
- Participates in the Continuum of Care Committee on a
twice-monthly basis
- Participates in Performance Improvement Committee as assigned by
DOS
- Assists in Resident Satisfaction surveys annually
- Interacts with residents and families/responsible parties and
staff; answers questions, gives direction and support
- Attends resident care plan meetings in SNF as appropriate
- Assures compliance with regulations including policy and
procedures, documentation and care practices.
- Promotes and protects the rights of each resident.
- Is a mandatory reporter for all suspected abuse situations
- Complies with all HIPAA standards of practice
- Provide input to the health care team on individual resident
status and safety.
Human Resources Development
- Participates in continuing education to maintain current
licenses
- Role models professional behavior and coaches/mentors staff to
develop maximum potential
- Recognizes nursing best practices and conducts training as
indicated
Fiscal Management
- Assists DOS in adhering to designed budget
Communication
- Works cooperatively with interdisciplinary staff to plan a
change in the levels of care for the safety SPT residents
- Represents facility in off-site and central office meetings as
assigned by DOP
- Able to demonstrate ability to communicate effectively with
staff, colleagues and supervisors
Safety
- Adheres to Fire Life Safety codes/regulations
- Participates in Facility Emergency Plan training and fire drills
and if an emergency or disaster occurs, immediately assumes the
pre-assigned emergency duties
- Role models work tasks safely and in compliance with facility
safety program
- Other duties as assigned by Director of Outpatient Services.
Required Qualifications:
- Licensed RN or LVN in the State of California.
- Current Cardio Pulmonary Resuscitation Certificate
- Three to five years nursing experience in geriatric population
- Ability to read, speak well and be understood in the English
language
- Satisfactory completion of physical examination including TB
test
- Fingerprint clearance from Department of Justice
- Computer literacy
Preferred elements:
- Advanced degree in health care management and/or nursing
desired.
- RCFE certificate preferred
- Bilingual ability
Exposure Risk
The Case Manager is at high risk for exposure to blood
and body fluids if actively providing nursing care to residents.
Performance of administrative duties results in minimum exposure.
Physical/Sensory Requirements: (with or without
corrective devices)
- Able to move continually throughout the facility during the work
day
- Able to cope effectively with physical and mental performance
elements
- Demonstrate emotional stability
- Able to lift, push, pull up to 50 pounds
- Dexterity to pinch small objects
- Vision adequate to read medical instruments, medical record
- Hearing adequate to communicate with peers and residents
